Please be sure to review the request(s) your child would like to make before submitting. This form is only accessible through a student’s Okemos Public Schools Google account. This form can only be completed once but you will be able to edit your submission.

Schedule change request forms must be submitted by August 11, 2019 via the 2019-20 Schedule Change Request Form which can be accessed at https://forms.gle/FYSHwFtMi8TjKSNDA. Please note that you will not be able to schedule a drop/add appointment without completing this form. Please follow the directions on each page carefully.

For specific information about the Okemos High School drop/add policy,  please refer to the OHS Student-Parent Handbook (the section titled “Classes”) or to the OHS website’s Parents & Students tab.  Familes can refer to the Master Schedule for the latest version of next year's schedule and the Guide to Curriculum for course descriptions with prerequisite information.
